Quick Start
Create and publish your first SK Form Builder form in under five minutes.

This guide walks you through building a basic contact form and adding it to your Shopify store.
1. Create a new form
Open the app
Go to your Shopify Admin → Apps → SK Contact Form Builder.
Click Create Form
On the dashboard, click Create Form. Choose Start from scratch or pick a template from the library.
Give it a name
Enter a name for the form (e.g. "Contact Form"). This name is internal customers don't see it.
2. Add fields
The form builder opens with a blank canvas.
Open the Fields panel
Click the Fields icon in the left sidebar to see all available field types.
Drag fields onto the canvas
For a basic contact form, drag in: Text (Name), Email, Textarea (Message), and any other fields you need.
Configure each field
Click a field on the canvas to open its settings. Set the label, placeholder, whether it's required, and any other options.
Width control
You can set each field to 33%, 50%, or 100% width to place fields side by side on the same row.
3. Set up email notifications
Open Settings panel
Click the Settings (gear) icon in the left sidebar.
Configure admin email
Under Email Notifications → Admin Email, enter the email address that should receive a notification on each submission.
(Optional) Enable customer email
Toggle on Customer Email to send a confirmation email to the person who submitted the form.
4. Configure after-submission action
In the Settings panel, scroll to After Submission Action and choose what happens when the form is submitted:

| Clear & Show Message | Default clears the form and shows a success message |
| Redirect to URL | Send the customer to a thank-you page or another URL |
| Hide Form & Show Message | Hide the form entirely and show a message in its place |
Customize the Success Message text to match your brand.
5. Design the form
Open Design panel
Click the Design (palette) icon in the left sidebar.
Choose a layout
Select Default (inline) or Boxed for a card-style form. Popup and floating layouts require Basic+ or Pro+.
Adjust colors and spacing
Set your button color, input border color, background, and font to match your store's branding.
6. Save and publish
Click Save in the top-right corner of the form builder. The form is now saved and ready to embed.
7. Add to your store
Click Add to theme
From the form builder or the dashboard, click Add to theme.
Place the block
The Shopify Theme Editor opens. Drag the SK Form block to your desired page or section.
Save the theme
Click Save in the Theme Editor. Your form is now live.
Your form is live!
Visit your store and test the form end to end. Check that submissions appear in the app dashboard under Form Responses and that notification emails are delivered.
