Slack

The Slack integration posts a message to a Slack channel when a form is submitted. Get real-time alerts in the channel your team already monitors.

Setup
Create a Slack Incoming Webhook
In Slack, go to Apps → Incoming Webhooks → Add to Slack. Choose the channel where messages should appear. Copy the Webhook URL Slack provides.
Add the webhook URL to Global Settings
In SK Form Builder → Global Settings → Slack:
- Toggle Enable Slack on
- Paste the Webhook URL
- Save
Enable on the form
In the form builder → Settings → Integrations → Slack → toggle Enable Slack on and save.
What the Slack message contains
The Slack message includes:
- Form name
- Submission timestamp
- All submitted field values formatted as a readable list
- A link to view the response in the app dashboard
Different channels per form
The Slack integration uses a single global webhook URL (one channel). To send different forms to different channels, create separate Incoming Webhooks in Slack one per channel and note that currently the global webhook is shared. For per-form channel routing, use Zapier with a Slack action.
Combine Slack notifications with email notifications. Slack is great for real-time team awareness; email is better for records and off-hours alerts.
